The SIP Game The game is aimed at you who are: residents and offered a coordinated individual plan, SIP, or responsible for SIP or curious about what SIP is.The purpose of the SIP game is that through gamification, more easily accessible and fun-filled, you get an understanding of how the SIP process is implemented in depth.
The understanding takes place in an interactive way, like a digital role-playing game which takes place in the mobile when and where you want.In the game you get to meet different fictional people who are in need of a SIP.
The people have different needs and you get different roles based on who pays attention to the need for SIP.You will go through the whole SIP process.
The process takes place in six moments; discover needs, prepare, call / invite to SIP meeting, conduct the meeting, follow up and end.
Often several follow-ups take place on a SIP.
A SIP can exist for a shorter or longer period of time and be more or less extensive.You can try different types of SIPs, more will appear in the meantime.
Then you will see how satisfied everyone involved is with your effort, maybe you can do better from yourself in the future?In the game there is an avatar that gives you tips and tricks in the SIP process and presses important parts of the process.
If you want to learn more about SIP, there are movies, podcasts and other support material available in the game.
There are also Guidelines for SIP that apply in Västra Götaland.Played hatred developed in the assignment "Strengthened work with SIP in Västra Götaland" in collaboration with dimh AB.
The game is funded by city grants for mental health 2020, through the Knowledge Center for Mental Health Västra Götaland region.
Briefly about SIP: Coordinated individual plan, SIP is a document and a tool for collaboration, which provides a comprehensive description of the individuals ongoing and planned efforts, from the municipality and region and other actors.Everyone should have the opportunity to live a safe, meaningful and independent life.
Specialized systems in health care, social services and schools place great demands on coordination.
Work methods that create clarity and overview lead to improved patient safety and time savings.SIP shall provide an overall picture and simplify for those concerned who can quickly get an overview of all ongoing efforts for the individual.
SIP applies where the need for co-planning may arise between the municipality and the region and in connection with hospital stays.The target group for SIP is everyone, regardless of age, diagnosis, functional variation or need.When the individual feels that it is unclear what different activities do or plan to do, it may be relevant to have a SIP - SIP is the individuals plan!The purpose of SIP is for the individual to have an influence and be involved in the planning and implementation of the health, care and nursing that is needed.
The efforts must be offered early and it must be clear to the individual and relatives as well as to the businesses.
SIP and the SIP process should • create participation and influence for the individual.• start from what is important to the individual and be the individuals plan.• contain what the individual can do themselves, what they need help and support with, who is responsible for what and when, and which contact persons the individual has in the various activities.• form an understandable, manageable and meaningful whole for the individual, any related parties, activities and other relevant actors.
Fixing SIP Provisions on SIP have been in the Social Services Act and the Health Care Act since 2010.
The plan must be drawn up when the individual needs efforts from both the social services and the health care system, provided that the plan is needed for the individual to have their needs met and that it individual himself agrees to the plan.SIP is also regulated in the Act on Collaboration in Discharge from Closed Health Care, which entered into force in 2018, and for discharge from psychiatry in January 2019.
The purpose is to promote good care for people who need interventions from both principals after discharge from closed healthcare.
